401(k) Plan First of the month following hire date Employees can contribute up to 60% of salary up to the annual IRS elective deferral limit Company matches 15% of the employee contribution (up to the first 15%), fully vested after 3 years of service.

Employee Stock Ownership Program

Must meet "years-of-service" requirements The plan is funded entirely by the company Discretionary contributions once a year depending on profitability of the company

State Disability

At time of employment Company and employee pay

State disability benefit payments begin after 7 consecutive days of disability

Long-Term Disability (Optional) First of the month following hire date Employee pays 50% 60% of monthly salary to maximum of $7500 per month after 30 days of continuous disability to age 65
Personal/Sick Time Accrued from first full payroll period after hire date Company pays 100% 5 days per year, may be accumulated to a maximum 30-day carryover
Vacation Time Accrued from first full payroll period after date of hire Company pays 100%

Based on service with company:
1 – 5 years = 10 days
6 – 10 years = 15 days
1 additional day per year to 20 days

Holidays At time of employment Company pays 100% 8 paid holidays per year
Tuition Reimbursement After 3 months of employment, at completion of job-related course or degree program, and company approved

Company reimburses based on grade:
A: 80%
B: 65%
C: 50%

Tuition and lab fees for approved courses

Flexible Spending Plan New hires can enter monthly; employees can join annually during open enrollment Employees contribute through payroll deduction Pre-tax contributions can be used for out-of-pocket expenses, such as deductibles, vision care, well care, prescriptions, and dependent care service
Employee Assistance Program (EAP) Available to all employees and their immediate family The cost of EAP is paid by the company; costs beyond the services of the EAP shall be the responsibility of the employee
